New Patient Forms
You can print forms and fill them out at your convenience before your first visit. Or you may fill them out in our office if you prefer.
Appointments and Cancellations
Our staff believes in courtesy and convenience for our patients. We do all we can to schedule around your life, and confirm appointments prior to visits. However, we ask for a 48-hour notice from you, if possible, if you need to change or cancel an appointment.
Your First Visit With Mai Dentistry
The first time you visit Mai Dentistry, we know that you will enjoy it. Our friendly front desk staff tries their best to help you feel welcome and at home.
We keep a light and fun mood for children. When we’re ready for you, we’ll bring you back to one of our rooms and allow you to get comfortable. Once you’re comfortable, we will discuss your dental history and any concerns that you may have. The more information you share, the better chance we have to give you optimal, comfortable care. The assistant will then take x-rays and pictures of your teeth before Dr. Mai performs a thorough examination, including an oral cancer screening, and inspecting for tooth decay and gum disease. She will discuss findings and explain concerns that may need further consultation. We realize that this is an important moment; communication between patient and dentist is key! Finally, if no signs of gum disease are present, your biannual cleaning will be performed.
We give you the chance to schedule your next visit when you check out. If you prefer to wait, you will be called and reminded when it’s time—twice yearly. See you next time!
